For some people, it’s nearly an automatic answer when they’re asked that question. So, why?

I was asked that question earlier today by a passing acquaintance of mine while I was waiting for class to start. Even though I’d spent the last four days suffering from side effects of my increased dosage of medicine, I instantly answered, ‘I’m doing fine, how ’bout you?’.
